Extended hours are offered as needed * after hours appointments available
Service Description: Community support to adults who have co-occurring diagnoses of serious mental illness (schizophrenia or other psychotic disorder, bipolar disorder, or major depression with psychotic features) and substance use disorders
  • one-one counselling (in-person, virtual or phone appointments)
  • trauma informed assessment, treatment planning, and counselling
  • education and clinical consultation
  • connection and collaboration with other support services as needed
  • case management, advocacy, case conferencing and referral
  • group therapy

Appointments available in Owen Sound, Hanover, Kincardine, Markdale, and Southampton

Fees: Free
Application: Self or service provider referral * referrals are received and screened centrally through the Owen Sound site
Eligibility / Target Population: Adults 18 years and over with both a serious mental illness and a substance use disorder (concurrent disorder) * must be willing to participate in the program and engage with counsellor

Language Notes: Attempts will be made to provide service in other languages as requested
