Service Description:

Mental health case management to people not able to receive support by existing community resources
Services include:- Time limited, goal-specific community treatment
- Assistance with medication issues
- Relapse prevention
- Psychoeducation
- Liaison with community organizations and advocacy

Fees:

Most fees are covered by OHIP

Application:

Referrals can be made by doctors, social workers, case workers, nurses, community agencies, and families
|
|
Eligibility / Target Population:
|
Youth 16 years and older and adults up to 65 years with serious mental health issues
