Service Description:	Mental health case management for people not able to receive support through other community-based resources time-limited, goal-specific community treatment assistance with medication issues relapse prevention psychoeducation liaising with community organizations and advocacy
Fees:	Most fees are covered by OHIP
Application:	Referrals can be made by doctors and social service agencies * call to initiate self-referral
Eligibility / Target Population:	People 16 years of age and over seeking treatment for moderate to serious mental health challenges
