Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness and provide preventative care * team includes family doctors, nurses, nurse practitioners, registered dietitian, social worker, mental health worker, psychologist, pharmacist and other health care providers and agencies in the community * seasonal flu vaccine available by appointment
