Service Description:	Supports for youth who are transitioning out of child protection services * individual and group sessions on: housing education employment life skills and healthy relationships money management physical and mental health services legal services goal setting system navigation and referrals

Eligibility / Target Population:	Youth 16-24 years who are current or former Crown Wards, or were in a formal customary care agreement immediately before 18th birthday * youth who were eligible to receive Renewed Youth Supports (RYS) at ages 16 or 17, or have a Voluntary Youth Services Agreement (VYSA) with Family and Children's Services St Thomas-Elgin
