Grand Bend Area Community Health Centre

The Grand Bend Area Community Health Centre is one of 101 Community Health Centres in Ontario that practice a common model of health and well-being for all. Two sites offer primary care, interprofessional services and team-based care to area residents on weekdays by appointment.

Position:  Family Physician

Region:  London and Middlesex, Huron and Perth

Description:  The Grand Bend Area Community Health Centre (GBACHC) is seeking a 1.0 FTE family physician to join our dynamic, interprofessional primary care team. Located along Ontario’s beautiful Bluewater coast, the GBACHC provides comprehensive, team-based care to diverse populations across Grand Bend, Hensall, and surrounding communities. We are committed to advancing the Quintuple Aim: improving patient and provider experience, health outcomes, equity, and value for money.

As a family physician at the GBACHC, you will work in a collaborative model alongside Nurse Practitioners, Registered Practical Nurses, dietitians, social workers, physiotherapists, Occupational Therapists, health promoters, and other inter-health professionals. Physicians at the GBACHC play a key role in:

  • Providing comprehensive, evidence-based and holistic primary healthcare, following the Health Disciplines Act of Ontario, the College of Physicians and Surgeons of Ontario (CPSO) practice standards, and Community Health Centre (CHC) guidelines
  • Supporting chronic disease management, preventative care, and health promotion initiatives
  • Participating in interprofessional case conferencing and quality improvement initiatives
  • Contributing to the teaching and mentorship of learners when applicable.

Qualifications: 

  • Independent practice license with the CPSO
  • Certification in Family Medicine (CCFP) from the College of Family Physicians of Canada (CFPC) or equivalent experience is an asset
  • Strong commitment to interprofessional, team-based primary care
  • Experience and/or interest in community health, chronic disease management, and preventative care
  • Demonstrated commitment to equity, diversity, and inclusion in healthcare
  • A completed Vulnerable Sector Police Check completed within the last six months

Compensation and Benefits: 

The GBACHC offers a competitive compensation package of $154.13 per hour, along with the following:
