Our mobile health services include a team of social services workers and clinical staff who partner on outreach to provide episodic care in the community. Travelling by SUV, on foot, or with our Clinic on Wheels, the team brings care and connection to London’s hardest to reach with the goal of diverting visits from Emergency Departments. This program is a collaborative effort between CMHA Thames Valley, London Cares, Regional HIV/AIDS Connection and London InterCommunity Health Centre.

Position:  Registered Nurse, Health Outreach Mobile Engagement

Region:  London and Middlesex

Description:  The Registered Nurse works as a member of an interdisciplinary care team providing episodic care, wound care, basic needs, and service connection referrals. The clinical team is a mobile team that works in various areas in the city with individuals that have experienced barriers accessing care and services. The team has quick access to refer to in-house Social Workers, Outreach Workers, Community Developers, and support staff specializing in the areas of Mental Health, Systems Navigation, Diabetes and Chronic Diseases while out in the community.

The compensation package: 

  • A starting salary range of $69,000 - $78,000

Qualifications:

Required:

  • Post-secondary diploma (RN) or degree (BScN) from a recognized college or university
  • Registration in good standing with the Ontario College of Nurses
  • Minimum 2 years of primary care experience
  • Awareness and understanding of the social determinants of health and health promotion
  • Ability to practice from social justice, trauma and violence-informed care, and harm reduction frameworks
  • Clinical knowledge of mental health care, substance use, concurrent disorders, dual diagnoses, and application of current best practices
  • Demonstrated effective crisis prevention, intervention, and stabilization skills
  • Ability to work with individuals from diverse backgrounds through an inclusive and anti-oppressive lens

Preferred: 

  • Experience in mental health care and working with people experiencing substance use disorders and homelessness
  • Demonstrated phlebotomy skills
  • Experience with Electronic Medical Records (Telus Practice Solutions Suite currently in use)
  • French and other language skills are considered an asset in supporting the communities we serve


French Proficiency:  French an asset

Salary:  $69,000 - $78,000 annually
