Central Community Health Centre (CCHC) is a charitable organization whose purpose is the delivery of comprehensive primary care, illness prevention, harm reduction, and health promotion through community programs and support to individuals and families in St. Thomas, Central Elgin and Southwold Township. Position:  Nurse Practitioner, Mobile Health Unit

Region:  Elgin

Description:  We are a Community Health Centre with our main office in St Thomas, Ontario, however, we want to get our mobile unit on the road to serve our community better (St Thomas, Central Elgin, and Southwold). This position would be ideal for a Nurse Practitioner who loves independence, meeting people where they are at and building innovative care spaces.

Our area is primarily rural and that is the population we are seeking to serve in a new and exciting way. While you may be on the road much of your week, you also have the full support of a multidisciplinary team with Central CHC. If this catches that pioneering bug in you, please let us know!

Responsibilities:

  • Deliver high-quality patient care in a mobile setting, including assessments, diagnoses, and treatment plans.
  • Perform routine check-ups, health screenings, and follow-up visits.
  • Collaborate with a multidisciplinary team to ensure comprehensive care.
  • Educate patients on health management, wellness, and preventive care.
  • Maintain accurate patient records and documentation in compliance with healthcare regulations.
  • Participate in community outreach programs to promote health awareness.

Qualifications:

  • Registered in the Extended Class with the College of Nurses of Ontario.
  • Has minimum three years’ experience in a community health setting, or a combination of community and hospital or public health settings
  • Has experience in program development, implementation, monitoring and evaluation.
  • Proficient in the use of computers and various software applications, including software used by the Ministry of Health and Long-Term Care, and other office equipment.
  • Confident working in a fast-paced environment and can respond in a positive manner to demanding issues while projecting a welcoming, friendly personality.
  • Low German speaker an asset
  • Experience in mental health an asset
  • Must have a valid Ontario Driver’s Licence
